What is square jaw surgery?
Square jaw surgery is also known as square jaw reduction is the surgery during which a wide and sometimes unsymmetrical jaw is trimmed into a slim and smooth one with spot-on proportions and symmetry. It is considered a major surgery thus it is important to find an experienced surgeon who will carefully carry out the operation minimizing muscle and nerve tissue damage.

When square jaw surgery is needed?
As we said before this is a major surgery (as most of the facial bone contouring surgeries are), and is usually done in case of the wide overdeveloped jaw bone with angular sides. Also if the face looks dull due to the developed bones, muscle and fat tissue along with the bone tissue, this surgery can reduce the width of the face as well. The square jaw surgery is a great help in improving facial asymmetry, but in that case, the operating surgeon must be highly experienced, cause this requires different techniques during the surgery. It also can be used along with the cheekbone reduction and chin surgeries to create a perfect v-line facial outline.
What are the methods?
Now there are 4 main methods that are used during square jaw reduction surgery and it depends on the facial bone structure, individual characteristics and desired result which of them is going to be used.
Long curve resection: this method is performed by gradually sloping resection of the lower chin from the area below the ear to the oral angle in a long curve. The square jaw from the front turns into a smooth line and makes a smooth chin line from the sides. The surgery can correct even severely asymmetric jaws. No visible scars are left due to incision inside the oral cavity.
Cortical bone resection: The bones in the body consists of 3 layers: outer cortical bone, cancellous bone, and inner cortical bone. The cortical bone we often refer to is the outer cortical bone with the strong bone tissue in the outer part. The cortical bone resection peels off some of the bone surfaces like peeling off a banana to make a slimming effect. It is only 2mm of the cortical bone to be peeled off for most people but it has a significant difference from the front side. Additionally, the surgery removes the developed muscles and fat for better results.
Muscle reduction surgery: This is a method using a small medical needle to transmit high-frequency waves to burn and reduce part of the muscles.
At this point, the number of muscle cells rather than its size is reduced and the surgery is semi-permanent because the degenerated muscle cells are never reproduced. The surgery reduces the thick muscles for overdeveloped masseter muscles from chewing to give a slimmer look.
Removal of deep facial fat: There is a long lump of fat from the temple beside the eyes to the side of the lips on the side of the face. It is called “isthmic fat” in medical terms and is also referred to as “deep facial fat” because the excessive amount of fat around the cheek makes the cheek look protruded. It is often seen during childhood but some remain even in adulthood. During the surgery, the fat is removed through the oral cavity. Excessive fat removal may damage nearby nerve tissues or salivary glands and make the cheek look sunken. Therefore, the exact position of the fat must be identified first and then 4–5cc of the fat must be carefully removed.
