What is the three-jaw surgery?

It is a combination of protruded mouth surgery and bimaxillary osteotomy that efficiently corrects complicated cases of a disproportionate face.

In some cases of severe protruded mouth and short chin, it is difficult to correct the facial balance with a general bimaxillary osteotomy or protruded mouth surgery. In these cases, three-jaw surgery provides efficient results.

The three-jaw surgery has more osteotomy steps than the bimaxillary surgery and is a difficult procedure that requires precision.

Surgery method

This surgery combines protruded mouth surgery and bimaxillary osteotomy to correct both the protruded mouth and short chin.

1. 4 premolars behind the canines are extracted and then the protruded gum bone is cut.

2. The gum bone and teeth are pushed to the proper position to correct the protruded mouth.

3. Bimaxillary osteotomy moves the receded lower jaw forward.

4. The chin tip advancement surgery is performed to move the tip of the chin forward.

Reasons why the three-jaw surgery is more effective than the Chin tip advancement surgery for the patient with severe mouth protrusion and short chin.

1. Chin tip advancement surgery only

Severe short chin condition is when the entire lower jaw is receded and has a short tip.

In this case, if only the chin tip advancement surgery is performed without bimaxillary osteotomy, only the tip of the chin becomes protruded while the lower jaw is still receded, thus forming an unnatural chin line.

2. Three-jaw surgery

The three-jaw surgery performing both the protruded mouth surgery and bimaxillary osteotomy simultaneously, thus correcting the protruded mouth and short chin as well as completing a natural chin line.
